St. Luke's Medical Requirements for Fiancé(e) visa

My Fiancee Visa Road-map to become Mrs. Edmiston 

Medical Examination at St. Lukes Medical Center (SLEC)
(Specific for Philippines K1 / K2 visa applicants)

Medical Date: Dec. 21-22, 2015 (K1 and 2 K2s) 
       K2s are 19 and 16 years old. 

Here are the requirements and specific documents that we brought: 

1.     St. Luke's on-line registration form. 
     This is helpful so that you don't need to fill-out the forms when you get there. Click here for the form: on-line registration form

2.     Passport, must be at least 6 months valid. If your Passport is approaching its 6 months validity expiry, renew your passport asap. The Embassy will require you a passport that is also at least 6 months valid. You don't want to be caught up with the delays in Passport renewal processing if you will be doing your interview. The Visa will be attached to your Passport, so it will be better that it is way valid for more years. 

3.     Copy of Passport (Bio page) 

4.     4 pcs. 2x2 photos, white background, shirt with collar, identical. If your photo was rejected there are Photo shops outside St. Lukes, of course it will be expensive (P100-120 for 6 pcs). There is one shop on the right side when you exit the bldg. after 7-eleven store. Only P80 for 8 pcs 2x2 photos. If you are not wearing a collared shirt, they can edit the photo for free. 

5.     Letter from the Embassy with Case Number (packet 3). If you have not received it yet, track your case at CEAC using your MNL number, screen shot and print that. CEAC Website. 
    Copies: K1- 2 copies; K2 - 1 copy each

6.     Interview confirmation letter (A MUST). SLEC requirements mentioned if available but the guard will be looking for this and will not let you in unless you have it. 
    Copies: K1- 3 copies (bring the original Embassy letter if you have it; K2- 1 copy each

7.     My Annulment papers/ Divorce papers of applicant (Copy). If you had a previous marriage and had an annulment or divorce bring your annulment papers. They will ask for this during Physical Examination, the doctor will read the terms of the annulment and will decide if you need a Psychological Consult.  I had an annulment but I was not referred to a Psyc consult, if your annulment papers says you suffered any abuse (physical, emotional, etc.) or depression; you will be referred to a Psychologist. Expect 1-2 weeks delay if with psyc consult; it will depend on the schedule of the psychologist. 

8.     Old chest X-ray film. This is just an additional reference, by the end of the day SLECs findings will suffice. 

9.     Results of our Annual Physical Exam (dated April 2015). Also for additional reference, but we don't need to show it. I just brought it to show that we have a good medical result based on our previous APE. 

10. Vaccine Records, me and the kids (including baby book)- ORIGINAL. This is very helpful especially for younger children. Vaccines are age specific, my 16 years old was given 3 vaccines despite we showed her vaccine records. Most of her vaccines were considered. 
    Copies: You just need one copy, I don't understand SLEC site mentioned 5 copies.

  5. Hello.. How are you? Ask lng sana ako about dun sa physical examination and immunization ng daughter nyo. Nabanggit mo kc sa blog mo na nkagat sya ng monkey. Ano ba sabi ng doctor during physical exam nang malaman nya nkagat ng money daughter mo? At nkapag vaccine ba sya after sya mkagat? Mgpapamedical din kc ako soon then nkagat din ako ng pusa years ago.. Sobrang worried ako sa magiging result ng medical ko dahil dun. Wait ko response mo. Thanks!,

    1. Sorry, I only saw your message now. You must be done with your medical by now. Everything should be fine, the cat bite will not affect that i'm sure.

  7. Hi. Im so glad I found your site. Its very helpful and up-to-date. Ask ko lang po, I also had an annulment and it said on my papers I was physically abused, but honestly hindi po, it was just written there para mapabilis ang annulment. My exhusband was psychologically incapacitated. Do you think SLEC will require me to do a psych eval even if I tell them i was not abused physically? Kasi I do not want to be delayed and I am also from Davao, going back and forth to Manila is expensive. Thank you...waiting for your reply.

  8. Hi! If Your papers says you were physically abused, stick to it. You already lied on that legal proceedings, you cannot undo that now for your convenience. They will ask if you were physically abuse, saying otherwise will only cause you more harm than good and may cause denial if brought up during the interview. I have no idea what's on your papers but the doctor will read it entirely and will ask you questions and may require you a psyc of eval. If your annulment was years ago, i.e. more than 10 years. If asked about the abused, you can say it was long time ago and you totally have move on from it. Your timeline from annulment plays bug factor. Again, never lie about domestic violence from then on. stick to what's on your papers than making a new story. Never, never mentioned thay you only did that to expedite uour annulmen during the interview. It will affect your credibility and may cause denial.

    Always be truthful.

    1. Thank you for your prompt reply. Yes, I will stick to what's written on my papers. My annulment was finalized August 2006 (10 yrs ago na), i will be going to SLEC this August. I hope I will not have to do psych eval anymore. God bless. You have helped a lot of people for the K1 process.

  9. Thanks sa response mo ha,sorry ngayon ko lng nabasa eh. Actually,d p ako nkapagmedical. Still waiting pa din ako sa case complete,under CR1 visa ako kya medyo matagal yung process compared sa K1. Still nervous pa din ako if ano maging result ng medical ko. Never naman ako nhospital kahit nung bata pa ako pero yung worry ko talaga yung cat bite kc kala ko baka mkaapekto sa medical ko. At salamat naman sa sinabi mo hindi sa maka apekto. Napapraning ako tungkol dun eh. haha

Hi first of all I want to thank you for all the work you did here to help those of us who are still going thru the process...I called the NVC this morning and got my case number, can my fiance schedule online the Medical at St Lukes now ? or does she still need to wait to receive something in the mail ?

  12. Hello! So I just called NVC and they finally assigned a case number for us yesterday. They just go our docs so the US Embassy in Manila has not received anything yet. Is there anything we can start now or do we have to wait until the Beneficiary received something from the Embassy? Thank you for your help!

Hello po, tanung ko lang po kasi po na approved na po kami thru email pero wala pa po yung hard copy ng NOA2 po tapos tinawagan po ng fiance ko ang NVC binigay na po sa kanya yung MNL case number namin, ano na po ang susunod na step na gagawin po namin? Kasi wala naman po kami hard copy ng NOA2 tska thru call lang po binigay yung MNL case number? pwede na po ba magbayad ng Visa fee kahit wala pa po kami hawak na mga hard copies? or mapapakita? yung MNL case number po nilista lang po ng fiance ko po...salamat po

In order to qualify for a fiancé(e) visa, you must intend to marry a particular U.S. citizen within 90 days of your arrival in the United States, you must be legally eligible to marry, you must intend to establish a bona fide marriage, and you must have met your intended spouse within the past two years. A K-1 is available only if your fiancé(e) is already a U.S. citizen. Lawful permanent residents (“LPRs”) or green card holders, cannot file for K-1 visas.
